{"id":683,"date":"2026-07-05T07:41:31","date_gmt":"2026-07-05T14:41:31","guid":{"rendered":"https:\/\/ccncn.org\/jt\/?page_id=683"},"modified":"2026-07-14T10:36:46","modified_gmt":"2026-07-14T17:36:46","slug":"live-scan-information","status":"publish","type":"page","link":"https:\/\/ccncn.org\/jt\/live-scan-information\/","title":{"rendered":"Live Scan Information"},"content":{"rendered":"\n<div class=\"wp-block-group alignwide has-global-padding is-layout-constrained wp-block-group-is-layout-constrained\">\n<p class=\"alignwide wp-block-paragraph\"><\/p>\n\n\n\n<h2 class=\"wp-block-heading has-large-font-size\">Process for Criminal Background Check Clearance (CALVECHS)<br><\/h2>\n\n\n\n<p class=\"alignwide has-medium-font-size wp-block-paragraph\">To be authorized to work with vulnerable populations, all candidates must complete a Department of Justice (DOJ) and FBI Live Scan fingerprint background check and a Mandated Reporter training.<br><\/p>\n\n\n\n<p class=\"alignwide wp-block-paragraph\"><a href=\"https:\/\/ccncn.org\/jt\/wp-content\/uploads\/2026\/07\/calvechs_clearance_checklist.pdf\" target=\"_blank\" rel=\"noopener\">Download the checklist<\/a><br><\/p>\n\n\n\n<p class=\"alignwide has-medium-font-size wp-block-paragraph\">Please follow these steps to complete your clearance.<br><\/p>\n\n\n\n<h2 class=\"wp-block-heading alignwide has-x-large-font-size\">Phase 1: Prepare Your Documents<\/h2>\n\n\n\n<p class=\"alignwide has-medium-font-size wp-block-paragraph\">Before visiting a Live Scan location, download and fill out the required paperwork.<\/p>\n\n\n\n<p class=\"alignwide wp-block-paragraph\"><\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li class=\"has-medium-font-size\"><strong>Download Your Forms:<\/strong><br>Download and print the forms required for your specific applicant type:<\/li>\n\n\n\n<li class=\"has-medium-font-size\"><strong>Fill Out the Live Scan Form:<\/strong><br>Locate the middle section labeled <strong>&#8220;Applicant Information.&#8221;<\/strong>\n<ul class=\"wp-block-list\">\n<li>Fill in all fields in this section completely (leave the &#8220;Billing Number&#8221; field blank).<\/li>\n\n\n\n<li>Stop when you reach the signature and date lines under the Privacy Notice section. <strong>Leave the signature and date blank<\/strong>\u2014you will sign this in front of the operator.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li class=\"has-medium-font-size\">Complete and sign the CORI Consent Form entirely.<\/li>\n\n\n\n<li class=\"has-medium-font-size\"><strong>Find a Live Scan Location:<\/strong>Locate an authorized agency.\n<ul class=\"wp-block-list\">\n<li>Find a nearby agency that provides background check services using the official <a href=\"https:\/\/oag.ca.gov\/fingerprints\/locations\">California DOJ Live Scan Location Directory<\/a>.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"has-medium-font-size\"><strong>Live Scan Request Form:<\/strong> Ensure the application type is verified for <strong>CALVECHS<\/strong>. Choose the correct version: <a href=\"https:\/\/ccncn.org\/jt\/wp-content\/uploads\/2026\/07\/BCIA_8016-Pre-filled-CCNC-N-VOLUNTEER_rev._3.2024.pdf\">Volunteer Form<\/a> or <a href=\"https:\/\/ccncn.org\/jt\/wp-content\/uploads\/2026\/07\/BCIA_8016-Pre-filled-CHURCH-EMPLOYEE_rev._3.2024.pdf\">Employee Form Link<\/a>.<\/li>\n\n\n\n<li class=\"has-medium-font-size\"><strong>CORI Consent Form:<\/strong> Download and print the Criminal Offender Record Information (CORI) Consent Form here: <a href=\"https:\/\/ccncn.org\/jt\/wp-content\/uploads\/2026\/07\/calvechs-bcia9018-Consent_Form.pdf\">CORI Form<\/a>.<\/li>\n<\/ul>\n\n\n\n<p class=\"alignwide wp-block-paragraph\"><br>Phase 2: Your Live Scan Appointment<\/p>\n\n\n\n<p class=\"alignwide has-medium-font-size wp-block-paragraph\">Bring your prepared paperwork and a valid government-issued photo ID (such as a driver&#8217;s license or passport) to your chosen location.<\/p>\n\n\n\n<p class=\"alignwide wp-block-paragraph\"><\/p>\n\n\n\n<h3 class=\"wp-block-heading has-large-font-size\">Background Check Fees<\/h3>\n\n\n\n<p class=\"alignwide has-medium-font-size wp-block-paragraph\">Fees are set by the state and the individual scanning agency, and are subject to change.
