Process for Criminal Background Check Clearance (CALVECHS)
To be authorized to work with vulnerable populations, all candidates must complete a Department of Justice (DOJ) and FBI Live Scan fingerprint background check and a Mandated Reporter training.
Please follow these steps to complete your clearance.
Phase 1: Prepare Your Documents
Before visiting a Live Scan location, download and fill out the required paperwork.

Download and print the forms required for your specific applicant type: - Fill Out the Live Scan Form:
Locate the middle section labeled “Applicant Information.”- Fill in all fields in this section completely (leave the “Billing Number” field blank).
- Stop when you reach the signature and date lines under the Privacy Notice section. Leave the signature and date blank—you will sign this in front of the operator.
- Complete and sign the CORI Consent Form entirely.
- Find a Live Scan Location:Locate an authorized agency.
- Find a nearby agency that provides background check services using the official California DOJ Live Scan Location Directory.
- Live Scan Request Form: Ensure the application type is verified for CALVECHS. Choose the correct version: Volunteer Form or Employee Form Link.
- CORI Consent Form: Download and print the Criminal Offender Record Information (CORI) Consent Form here: CORI Form.
Phase 2: Your Live Scan Appointment
Bring your prepared paperwork and a valid government-issued photo ID (such as a driver’s license or passport) to your chosen location.
Background Check Fees
Fees are set by the state and the individual scanning agency, and are subject to change. You will pay the agency directly at the time of service.

Note on Rolling Fees: “Rolling fees” are the local handling fees charged by the fingerprinting agency. You can find specific rolling fees for each location in the DOJ Location Directory.
At the Window
The Live Scan Operator will capture your fingerprints electronically, complete the bottom section of your Live Scan form, and hand a copy back to you. Keep this document safely.
Phase 3: Final Submission & Training
To complete your authorization file, submit your paperwork and complete the required safety training.
- Submit Your Paperwork
Send the following three documents to the Regional Office (Attention: Custodian of Records):- The completed copy of your Live Scan form returned by the operator.
- A copy of your completed CORI Consent form.
- A supporting document verifying your role (such as a job description, job application, or letter of volunteer agreement).
- Complete Mandated Reporter Training
All candidates must complete an approved online Mandated Reporter training course.- Recommended Training: We recommend the free online courses available at California Mandated Reporter Training.
- Submission: Once completed, download your Certificate of Completion and send a copy to the Regional Office.
Certificates remain valid for two years from the date of issuance.
